2017 Street Fair Upper Madison Merchants

Upper Madison Merchant Application
This form is only for businesses with a store front in the Upper Madison Area.  Upper Madison Business should be listed in our Business Directory.  If you believe you are an Upper Madison Business, but are not in the directory, please let us know at uppermadisonmerchants@uppermadison.org.

Fee:
  • Upper Madison Merchants pay no fee to participate in the fair.  Other vendors pay a fee of $40.
  • Any one selling food must provide information below for our Temporary Food Service License
  • Event will be cancelled in the event of heavy rain.
Space Details:
  • Space is 10’ x 10’, and located on the street (asphalt)
  • The fair provides space only. Merchants must provide everything else they need (tables, signs, etc.)
  • Pop-ups welcome.
  • No generators allowed
Setup:
  • Set-up no earlier than 10 a.m. and be ready by 11:30 a.m.
  • Event runs 12 to 5 p.m.
  • Merchants must be off the street by 5:30 so it can reopen to traffic.

All vendors must have a NYS Tax ID#. If you do not have a NYS Tax ID#, please call the NYS tax office at 518-485-2889 for a form.

There is limited space at the Upper Madison Street Fair. Help us understand how much space you need, and how you will help enliven the Fair, by answering the following questions.

Do you plan to sell or give away anything at the Fair? If so, please describe it here.
 
Activity
Do you plan to offer a free activity to help enliven the fair? If so, please tell us about it so we can highlight it in our marketing. In the past, people have provided facepainting, temporary tattoos, free massages, crafts for kids, guessing games and many other fun and interesting activities.

All food sold at the Fair is subject to Albany County Department of Health regulations; they are numerous! (for a complete list go to http://www.health.ny.gov/regulations/nycrr/title_10/part_14/subpart_14-2.htm#s1)

The most important requirement is that all food must be from approved sources, prepared in facilities under permit by the Albany County Health Dept. or an appropriate regulatory agency. Food may not be prepared at home.

It is up to you to meet these conditions; by selecting "Yes” above you acknowledge your responsibility for adhering to these policies.

Raffle Item
Much of the funding for the Fair and other Upper Madison Improvement Group activities comes from a raffle held the day of the Fair. We appreciate donations of items valued at $25 or more to be used in the raffle. If you are willing to donate a raffle item please describe it here.
