This form is only for Vendors who ARE primarily selling food for on-site consumption. (Other applications available here.)

We strongly prefer online applications. You can register online even if you want to pay by check. Online registration is the fastest and easiest way to register. If you are unable to register online, please contact us at vendors@uppermadison.org so we can assist you.
