On-Site Food Vendor Application
This form is only for Vendors who are primarily selling food for on-site consumption. (Other applications available here.)

We strongly prefer online applications. You can register online even if you want to pay by check. Online registration is the fastest and easiest way to register. If you are unable to register online, please contact us at onsitefood@uppermadison.org so we can assist you.

Fee:
  • Non-refundable $50 fee payable after approval   plus 10% of gross, payable at 5:00 PM on the day of the fair.
  • The fee is generally non-refundable. However, if we are legally unable to hold the Street Fair as a result of COVID-related or similar restriction, vendor fees will be refunded.
  • Fee includes Temporary Food Service License. If you are not already permitted by Albany County, please provide your workers compensation and disability forms as part of your registration.
  • Event will be cancelled in the event of heavy rain or dangerous weather.
Space Details:
  • Space is 10’ x 10’, and located on the street (asphalt)
  • The fair provides space only. You must provide everything else you need (tables, signs, etc.)
  • Pop-ups are encouraged. Please plan to weight your pop-up. It can be quite windy at the fair and unweighted pop-ups can be dangerous.
  • Generators are allowed at select spaces only. Please consult with us at onsitefood@uppermadison.org if you would like to bring a generator
Setup:
  • Set-up no earlier than 10 a.m. and be ready by 11:30 a.m.
  • Event runs 12 to 5 p.m.
  • You must be off the street by 5:30 so it can reopen to traffic
Other Policies:
  • Consistent with NYS law regarding "symbols of hate", vendors at the Upper Madison Street Fair will not be permitted to sell or display any item inscribed with a symbol of hate unless the image appears in a book or similar medium that serves an educational or historical purpose. The term "symbols of hate" shall include but not be limited to symbols of white supremacy neo-Nazi ideology or the Battle Flag of the Confederacy.

Food for Onsite Consumption
All food sold at the Fair for onsite consumption is subject to Albany County Department of Health regulations; they are numerous! (for a complete list go to https://regs.health.ny.gov/volume-title-10/1651173225/subpart-14-2-temporary-food-service-establishments). The most important requirement is that all food must be from approved sources, and be prepared in facilities under permit by the Albany County Health Department or an appropriate regulatory agency. Food may not be prepared at home.

It is up to you to meet these conditions; by submitting this application, you acknowledge your responsibility for adhering to these policies. Upper Madison Street Fair is required to include all food for on-site consumption to our temporary food service permit, so please provide a list of all foods you plan to provide for on-site consumption here.
 
Upper Madison Street Fair is required to include all food for on-site consumption on our temporary food service permit. If you are NOT already permitted by Albany County, please provide your: 1) Certificate of Workers Compensation Insurance (C-105.2, U26.3, SI-12, or GSI-150.2), and 2) NYS Disability and Family Leave (DB-120.1 or DB-155). If you ARE already permitted by Albany County, they will have these forms on file already, so you do not need to provide them here.
Power
We have a very limited number spots with power and your request here does NOT GUARENTEE that we will be able to provide power. If you have any questions about generators or power, please contact us at onsitefood@uppermadison.org before applying.
Raffle Item
Much of the funding for the Fair and other Upper Madison Improvement Group activities comes from a raffle held the day of the Fair. We appreciate donations of items valued at $25 or more to be used in the raffle. We may combine your item with other items to create appealing raffle baskets. If you are willing to donate a raffle item please describe it here.
Please enter the approximate dollar amount of your raffle item as a whole number with no dollar sign or decimal point. For example, if your item is worth fifty dollars enter 50.
